itself and tells the customer the condition is covered. The interpretation is
wrong, the customer proceeds with a costly procedure expecting coverage, and
the claim is later denied — creating both a customer harm event and a
potential regulatory complaint about unlicensed benefits interpretation.

- No abstention classifier or escalation trigger registry for decision categories requiring expert judgment (legal, medical, policy-exception).
