Issue: Agent acts in production instead of staging, wrong tenant, or wrong project.

Frequency: Rare but Catastrophic

Symptoms

  • Unexpected production changes or cross-tenant output.
  • A write/deploy call executes against production when the user’s stated intent and task context indicated staging (or a different tenant/project).
  • Agent’s session carries a stale or default environment value from an earlier, unrelated task rather than the one just declared by the user.
  • Single shared credential permits the call to reach both environments, so nothing blocks the mismatch at the auth layer.
  • No pre-execution echo/confirmation of the resolved environment or tenant ID exists before a destructive or write call proceeds.

Root Cause Because a single deployment credential accepts an environment parameter rather than being physically scoped to one environment, the agent’s choice of target is enforced only by its own reasoning, not by anything at the auth layer — a wrong guess simply goes through. That guess is especially likely to be wrong in long-running sessions, where environment or tenant context set for an earlier, unrelated task can silently persist and get reused for a new request without anyone re-confirming it is still correct. Nothing requires the agent to echo back and confirm the resolved environment before a write proceeds, and because the deploy tool itself is shared across environments (selected via a flag rather than being a separate, namespaced tool), environment selection is left as an agent-controlled parameter instead of a routing-layer decision — with no real-time check comparing credential scope against the resource actually being touched, the mismatch isn’t caught until after the change has already landed.

Expected Failure State

A configuration change intended for staging is deployed to production instead, affecting live traffic, because the agent’s stale environment context went unchecked against the user’s explicit “staging” instruction and the shared credential permitted the cross-environment write. A correctly defended system uses physically distinct per-environment credentials (a staging-scoped credential cannot reach production endpoints) and requires the agent to explicitly assert and match the target environment against the declared task context before any write proceeds.

Mitigation Strategies

Prevention

  1. Environment-Scoped Credentials, No Cross-Env Reuse: Issue physically distinct credentials/API keys per environment (prod, staging, per-tenant) rather than a single credential with an environment parameter the agent can set. A staging-scoped credential cannot reach production endpoints even if the agent’s reasoning about which environment it’s in is wrong.
  2. Pre-Execution Environment Assertion: Before any write/destructive call, require the agent to explicitly echo back the resolved environment and tenant/project ID it believes it’s operating in, matched against the session’s declared task context. Mismatches block execution rather than proceeding on the agent’s unchecked assumption.
  3. Environment-Namespaced Tool Registries: Expose staging and production as entirely separate tool registries/endpoints (not a shared tool with an “env” flag), so environment selection happens at the routing/deployment layer under human control, not as a parameter the agent can set incorrectly mid-task.

Detection & Response

  1.
