Issue: An Issue-Resolution Agent Authorized to Auto-Process Refunds Executes the Refund Directly Off the Customer’s Narrated Claim Without Calling the Ledger-Verification Tool It Has Access To, Because the Claim’s Phrasing Matches a Known-Resolvable Template

Symptoms

  • Agent processes a refund or credit within the same turn a customer states a claim (“I was charged twice,” “the item never arrived”), with no intervening call to the transaction-ledger or shipment-tracking tool that would confirm the claim
  • Execution trace shows the refund tool was called but the verification tool (available in the same tool set) was never invoked for that conversation
  • Refund-approval rate for claims matching common resolvable phrasing (“charged twice,” “never arrived,” “wrong item”) is measurably higher than the rate at which those claims are independently confirmed true when audited against ledger/shipment data
  • Chargeback and refund-abuse rates climb specifically for claim categories with high natural-language specificity (customers who describe the problem fluently and in policy-matching language) rather than correlating with actual claim validity
  • When the same claim phrasing is tested against a fabricated or already-refunded transaction, the agent still approves the refund, since nothing in its decision path depends on the ledger actually containing a matching, unrefunded charge

Root Cause The agent’s refund-decision step is prompted to recognize when a customer’s stated problem matches a known, policy-covered resolution pattern and to act on that match, but recognizing that a claim’s phrasing fits a resolvable category is a different computation from confirming the claim’s content against a system of record. Because the agent’s context does not distinguish claims by provenance — a customer’s free-text assertion sits in the same context as facts the agent could retrieve from a tool — a fluent, specific, policy-matching claim is sufficient on its own to drive the resolution-generation step into the “execute the compensating action” path, even when a verification tool exists and was never called to ground the claim before acting.
