Symptoms
- Action happens before prerequisite evidence exists.
- Refund, credit, or access-grant issued before the corresponding payment, identity, or authorization lookup ever runs.
- Agent cites a prerequisite as “checked” in its reasoning trace without a corresponding tool call or evidence record.
- Retried actions skip the prerequisite the second time because the agent assumes state from the first (failed) attempt still holds.
- Prerequisite step exists in the agent’s plan but executes after the dependent action rather than before it.
Root Cause Prerequisite checks like a balance lookup or a permission verification are usually treated as obviously implied by the task description rather than encoded as their own explicit, required steps, so nothing forces the agent to actually perform them before acting. The underlying API layer doesn’t enforce these preconditions either, so a skipped check produces no error at call time — the action simply succeeds, masking the gap — and time pressure in a short conversation further incentivizes shortcutting straight to the requested action. Because prior runs that skipped the prerequisite still “worked,” the shortcut gets reinforced as an acceptable pattern, and in multi-turn sessions the agent can lose track of which prerequisites were actually satisfied earlier versus merely assumed, letting a stale assumption stand in for a real check.
Example
A customer support agent handling a chargeback dispute is asked to "issue a full refund to the customer for order #48213." The agent calls the refund API directly using the order ID from the customer's message, without first calling the payment-verification lookup that confirms the order was actually charged (and not already refunded once). It turns out the order had already been refunded manually by a human agent the previous day; the automated agent's refund goes through anyway, doubling the payout. The missing prerequisite — a balance/charge-status lookup — was implied by the task but never made an explicit, checked step.
Contributing Factors
- Prerequisite lookups are treated as “obviously implied” by the task description rather than encoded as explicit required steps.
- The tool executor does not enforce preconditions at the API layer, so a skipped check doesn’t produce an error until much later (if at all).
- Time pressure or a short conversation with the user creates incentive to shortcut to the requested action.
- Prior successful runs without the prerequisite reinforce a pattern where the agent learns the shortcut usually “works.”
- Multi-turn sessions lose track of which prerequisites were already satisfied earlier versus merely assumed.

- Precondition Schema per Tool: Every tool/action definition declares a
required_preconditionslist (e.g.,permission_check,balance_lookup,user_confirmation). The tool-execution layer refuses to invoke the underlying API unless each precondition has a satisfying evidence record in the current session, making the check mechanical rather than dependent on the agent remembering to do it. - Evidence-Gated Tool Calling: Require the agent to attach a reference (a prior tool-call result ID or document ID) that satisfies each declared precondition when it proposes an action. Calls lacking valid evidence references are rejected by the executor before reaching the underlying system, not just flagged after the fact.
- Prerequisite Checklist Injection: For action types identified in a risk taxonomy as commonly missing prerequisites (payments, deletions, permission-gated reads), auto-inject an explicit “prerequisites” step into the plan template so the agent cannot propose the action without the antecedent steps already appearing earlier in the plan.
Detection & Response
- Precondition Violation Scanner: Middleware wraps every tool call and checks the evidence log for the immediately preceding precondition-satisfying calls; a missing entry blocks the call and logs a violation event with the tool name and missing precondition type.
- Trace Replay Auditing: Periodically replay sampled execution traces offline against the current precondition schema to catch violations that evaded live enforcement (e.g., due to a schema gap for a newly added tool).
- Outcome-Based Signal: Correlate downstream failures (permission errors, invalid-state errors, customer complaints) with sessions where a precondition was skipped, to prioritize which precondition gaps matter most.
