Insufficient Rollback
Issue: Agent cannot undo a bad action.
Frequency: Rare but Catastrophic
Symptoms
- No revert path after failure.
- Incident response stalls because engineers must manually reconstruct pre-action state from logs instead of triggering an automated undo.
- Partial rollback leaves the resource in a state that matches neither the pre-action nor the intended post-action state.
Root Cause The action was implemented as a hard, destructive operation — a permanent delete, a direct overwrite — with no compensating transaction defined and no pre-action snapshot captured specifically for the resource being modified, so once it executes there is no recorded prior state to return to. This typically goes unnoticed until a real incident because the rollback path, if one nominally exists, was never exercised in testing; for multi-step actions the gap is often partial rather than total — some steps have a defined compensation and others don’t — which is arguably worse, since it produces a resource stuck in a state matching neither the original nor the intended outcome.
Example
Agent hard-deletes 200 stale user records to "clean up test accounts" per a support request,
but the request actually meant a different, narrower set of records. There is no soft-delete
or snapshot for the delete operation, so the only recovery path is restoring from a nightly
backup — losing same-day writes for the affected accounts.
Contributing Factors
- Action implemented as a hard/destructive operation (permanent delete, direct overwrite) with no compensating transaction defined.
- No pre-action snapshot or backup captured specifically for the resource being modified.
- Rollback path was never tested, so it’s discovered to be broken only during a real incident.

Mitigation Strategies
Prevention
- Rollback Workflow Definition: For each action, define explicit rollback/compensation workflow. Document: what changes will be made → what compensating changes undo them → rollback execution steps. Pre-test rollback paths before deploying action in production.
- Reversible Defaults for Data Modification: Design all data modifications with reversibility in mind. Example: use soft-delete instead of hard-delete, store snapshots of modified records, implement point-in-time restore. Default to reversible operations.
- Compensating Transaction Pattern: For multi-step operations, define compensating transaction for each step. If operation fails mid-flight, execute compensation chain in reverse order to restore pre-action state. Test compensation paths regularly.
Detection & Response
- Rollback Failure Detection: Monitor rollback execution. Alert if rollback fails to restore expected state. Track: rollback_attempt, success/failure, time_to_restore, residual_state_delta. Log all rollback failures.
- State Restoration Verification: After rollback completes, query resource state and verify it matches pre-action snapshot. Flag rollbacks that fail to fully restore state (partial rollback). Escalate partial rollbacks.
- Unrevertible Action Tracking: Identify actions that cannot be rolled back (hard deletes, external API side-effects with no undo). Flag these as critical. Log all attempts to rollback unrevertible actions.
Architecture Patterns
- Snapshot-Based Rollback: For each action, create snapshot of affected resources pre-execution. Store snapshot in immutable store. On rollback, restore snapshot state. Enables point-in-time recovery.
- Compensation Transaction Registry: Maintain registry of all compensation transactions (reverse operations). Each action has associated compensation_transaction_id. Enable automated compensation on failure.
- Event Sourcing with Replay: Store all state changes as immutable events with reverse operations defined. Rollback by replaying events up to point before bad action, then applying compensation events.
