Issue: Agent says done when action was not completed.

Frequency: Common

Symptoms

  • No tool proof for claimed action.
  • Agent’s tool call returns an error or partial-failure status, but the response text still tells the user the action succeeded.
  • Customer contacts support again shortly after because the claimed action (refund, address update, cancellation) never actually took effect.

Root Cause Response generation is not gated on the tool-call result it’s supposedly reporting, so the model is free to emit a success phrase regardless of what the tool actually returned — a gap made worse by tools that signal failure ambiguously, such as an HTTP 200 with an error buried in the body, which the model doesn’t reliably parse. Without a post-action verification or read-back step, a call that fails silently downstream (accepted by the API but never persisted) has no chance to be caught before the confirmation goes out, and prompt guidance that rewards concise, confident closing statements gives the model every incentive to assert completion rather than flag missing or ambiguous evidence.

Contributing Factors

  • Response generation isn’t gated on the actual tool-result payload, so the model can emit a success phrase regardless of what the tool returned.
  • Tools return ambiguous or inconsistent success/failure signals (e.g., HTTP 200 with an error field buried in the body), which the model doesn’t parse correctly.
  • No post-action verification/read-back step, so a call that fails silently downstream (accepted but not persisted) is never caught before the confirmation message is sent.
  • Prompt rewards concise, confident closing statements, biasing the model toward asserting completion even when tool evidence is missing or ambiguous.

Mitigation Strategies

Prevention

  1. Completion-evidence requirement (tool-proof gating): block the agent from emitting a “done” message unless a corresponding successful tool-call result exists in the current turn’s context, since the failure is the agent asserting completion without grounding it in actual tool execution evidence. Trade-off: requires strict prompt/response-schema enforcement and can produce awkward responses when a tool legitimately has no return value to point to.
  2. Structured action-result binding: require the agent’s completion claim to reference a specific tool_call_id and its result payload (confirmation number, status field) rather than free-text assertion, so completion claims are programmatically verifiable. Trade-off: adds schema/output-format complexity and forces every actionable tool to return a machine-checkable success indicator.
  3. Idempotent verification call before claiming done: for critical actions, require a follow-up read/verification call (re-fetch order status) before the agent is allowed to state completion, since a tool call can appear to succeed at the API level but not actually persist the change. Trade-off: doubles latency and API load for every critical action.

Detection & Response

  1. Completion-claim-without-tool-call scanning: automatically scan transcripts for completion language (“I’ve done…”, “this has been updated…”) with no preceding successful tool call in the same turn, the exact behavioral signature of the failure. Response: flag for immediate transcript review and, if confirmed, notify the affected customer proactively.
  2. Downstream state reconciliation: periodically reconcile claimed actions (e.g., “refund issued”) against actual backend state (payment ledger) to catch false completions that evaded transcript-level detection. Response: any mismatch triggers a customer-facing correction workflow.
  3. User follow-up complaint correlation: correlate “this still isn’t fixed” or repeat-contact-on-same-issue signals with prior completion claims in the same thread. Response: route matched cases into the false-completion audit queue.

  1. Tool-result-gated response generation: architect the response generator so completion-phrase templates are only reachable from a code path with a verified successful tool-result object in scope, making a false completion claim a type error rather than a possible model output.
  2. Verification-before-confirmation pipeline: a two-step pipeline where step 1 executes the action and step 2 independently verifies state before step 3 generates the user-facing confirmation, structurally separating “I called the tool” from “the tool call is confirmed successful.”
  3. Completion audit log with replay: log every completion claim alongside the tool-call trace that justified it, enabling automated nightly replay/verification jobs that catch false claims that occurred in production.
