- Re-running the same order through the risk engine with the timeout resolved produces a genuinely different verdict (often a breach) than the one the agent had fabricated
- The order proceeds to submission because the agent’s fabricated verdict was “pass,” even though no real risk check was ever completed for that order
- Logs show no retry attempt and no explicit error-handling branch was triggered; the agent’s context simply contains a tool-call entry with an error status immediately followed by reasoning that treats a risk verdict as already known
Root Cause A model conditioned on a long history of successful risk-check tool calls has a strong prior over what the next turn “should” contain once a risk-check tool has been invoked: a structured verdict. When the actual tool response is an error or timeout rather than the expected schema, next-token generation continues to complete the expected pattern — a plausible-looking risk-check payload — rather than treating the error as a distinct case requiring a different response type (retry, escalate, halt). This is a generation-completion failure specific to agentic tool use: the model is not reasoning about whether the tool succeeded, it is pattern-matching the shape of “what comes after a risk-check call” and filling that shape in even when the actual tool output does not contain the data needed to do so honestly.
